Use this form if you are making a change to an already submitted Building Use Request Form.  This is not to request a new building use.

This change form should be submitted only by the person listed as the Primary Contact or the Secondary Contact on the original Building Use Form.

NOTICE:  Changes to the event date, times, or rooms are subject to approval.

Tip: Please, only list changes to the event.
